Joseph F Ferstl DC FACO Director at Large, Illinois Chiropractic Society Second Vice President American College of Chiropractic Orthopedics 1557 Weatherstone Ln Elgin  IL  60123 847-741-3355 847-741-3597 fax info@ferstlchiropractic.com Copyright 2010 Ferstl Chiropractic Clinic, P.C.         Web Design and Hosting by MEMNetService Thank you for choosing us as your chiropractic health care provider.  We are committed to successfully treating you and establishing a long-term relationship for excellent health.  Please understand that payment of your bill is considered a part of your treatment.  The following is a statement of our financial policy.   All patients must complete the "Patient Registration Form" before seeing the doctor.   We do work with many insurance plans and our office will assist you in billing your claims.  We are participating providers for most PPO plans.  We are required by most of our insurance agreements to collect your copay and copy of your insurance card, in order to verify eligibility of benefits.  Chiropractic care is also covered by many private health insurance plans, including Medicare.   Our practice will require you to assign all insurance carrier payments directly to our office.  Any portion of services rendered, which are denied or not paid by your insurance carrier, will become your responsibility.   Your insurance coverage is a contract between you and your insurance carrier; however, we will assist you to maximize your insurance benefits.  If an insurance problem occurs, you will be asked to assist us in contacting your insurance carrier.  We feel that it is financially beneficial to you if we work together to resolve any insurance issues.   Our practice firmly believes in establishing excellent doctor/patient relationships, which are rooted in good communication and understanding.  Missed appointments hinder that relationship.  As a courtesy to Ferstl Chiropractic Clinic's patients and staff, please give 24-hour notice for all cancelled appointments if it is within your control.   For your convenience, we accept Visa, MasterCard and Discover, in addition to cash or check.   We understand that some patients may urgently need chiropractic care, but are unable to pay practice fees due to financial hardship and/or lack health insurance.  Under those circumstances, special payment and/or a cash discount agreement can be established between the patient and Ferstl Chiropractic Clinic, P.C.   I Copyright 2010 Ferstl Chiropractic Clinic P.C.          Web Design and Hosting by MEMNetService Financial Policy
